Heart Failure (HF) Academy For Pharmacists

Introduction
Held at Cleveland Clinic Abu Dhabi, this training program delivers high-quality, cost-effective professional development and networking opportunities. We believe that the best way for pharmacists and other healthcare professionals to understand and implement current best practices in heart failure, is to be immersed in a setting that provides access to a comprehensive curriculum in a real-life setting with hands-on practice mentored by faculty who are subject matter experts.

Goals of the HF Academy:

  • Provide the foundational knowledge needed to optimize medications for heart failure patients
  • Provide the necessary knowledge and skills needed to implement transition of care services in heart failure
  • Expose trainees to state-of-the-art strategies and technology used in heart failure
  • Demonstrate how to evaluate the effectiveness of a initiatives and interventions in HF through measurement of outcomes

Overview

The heart failure (HF) Academy exposes trainees to current best practices through didactic lectures delivered by experts in the field, as well as hand-on training utilizing state-of-the-art technology. The program curriculum is spread over a two-week, 10 days period. The first week is dedicated to foundations of implementation of management of heart failure and the second week builds on the first week and provides more advanced knowledge. Throughout the two-week period, trainees will get the chance to participate in multidisciplinary HF rounds and interface with various healthcare providers to optimize medications prescribed under the preceptorship of experts in the field.

  • Heart failure patient assessment
  • Heart Failure Medications pharmacology
  • Pharmacokinetics and pharmacodynamics of HF meds and their clinical application
  • HF medications titration to target doses
  • Management of acute decompensated heart failure
  • Advanced HF management
  • Management of patients with left ventricular assist device
  • Measuring and reporting outcomes of HF transition of care (TOC) initiatives
  • Implementing HF TOC strategies

HF Academy Program Format
The program includes a didactic component which provides the foundational knowledge needed in the care of patients with heart failure. The program also has a hands-on training component which allows for the implementation of the foundational knowledge during the training period. Trainees will be attending daily multidisciplinary team rounds and provided the chance to work on state-of-the-art clinical decision support system.

Application Requirements
Applicants will be required to submit the program online application in addition to the necessary supplemental documents. Vital to their application submission, applicants must provide health clearance evidence from their home institution. The health clearance should include up to date vaccination records, negative TB results, as well as CCAD occupational health requirements. Overall, applicants must provide the following:

Dr. Atallah is the Heart and Vascular Institute pharmacotherapy specialist at Cleveland Clinic Abu Dhabi. Prior to joining Cleveland Clinic, he was on faculty as Assistant Professor of Clinical Pharmacy at King Saud bin Abdulaziz University for Health Sciences (KSAU-HS) in Riyadh, Kingdom of Saudi Arabia.Dr. Atallah received his PharmD from Bernard J. Dunn School of Pharmacy in Virginia and completed an ASHP accredited post-graduate residency at Winchester Medical Center in Virginia.

Dr. Atallah is board certified in pharmacotherapy with added qualifications in Cardiology. He also holds an advanced antithrombotic management certificate from the American Society of Health-System Pharmacists. He is a member of the American College of Cardiology, the American College of Clinical Pharmacy and serves on the Cardiology Practice and Research Network (PRN) Research and Scholarship committee, Heart Failure Society of America (HFSA) fellow and Middle East Ambassador, and American Society of Health-System Pharmacists. He also serves on the Health Authority Abu Dhabi (HAAD) subject matter experts (SME) pharmacology committee. He has several publications and presentations at national and international conferences. In his current role at Cleveland Clinic Abu Dhabi, Dr Atallah established the first per protocol physician-pharmacotherapy specialist heart failure collaborative practice agreement clinic in the Middle-East Gulf region.

Ziad Sadik is a Senior Pharmacotherapy Specialist in Cardiology and Internal Medicine. He received his Doctor of Pharmacy degree in 2011 from the Lebanese American University in Lebanon. He then completed a PGY1 Pharmacy Practice Residency at Henry Ford Hospital in Detroit, Michigan. Following the completion of his residency, he continued to work at Henry Ford as a clinical pharmacist until October 2014.

Ziad is board certified in pharmacotherapy as well as in cardiology pharmacotherapy and certified in Epic – Willow Inpatient. He is currently an adjunct clinical assistant professor at the Gulf Medical University and the United Arab Emirates University. His interests include anticoagulation management, cardiovascular risk reduction, formulary management as well as optimization of medication records in computerized physician order entry (CPOE) systems.

Feras Bader, MD, is a Staff Physician in the Heart & Vascular Institute at Cleveland Clinic Abu Dhabi. Prior to joining Cleveland Clinic Abu Dhabi, Dr. Bader served as Medical Director of the Heart Failure Program and Director of the Advanced Heart Failure and Transplant Fellowship Program at the University of Utah Hospital in Salt Lake City, Utah, US

He was also Associate Professor of Medicine and Cardiology at the University of Utah, specializing in the field of Advanced Heart Failure and Transplantation. Dr. Bader also helped establish an advanced heart failure program for the Kuwait Ministry of Health, adding valuable regional experience to his experience practicing in the US. Dr. Bader earned his medical degree from the University of Jordan School of Medicine in Amman, Jordan. He completed his residency in Internal Medicine as well as a fellowship in Advanced Heart Failure and Transplant at the Yale University School of Medicine in Connecticut, US. He then completed a fellowship in Cardiovascular Medicine at the University of Utah School of Medicine, where he also obtained a Master’s Degree in Clinical Investigation.
Dr. Bader is American Board certified in Internal Medicine, Cardiovascular Diseases, Echocardiography, and Advanced Heart Failure and Transplant Cardiology. His research interests include various aspects of ventricular assist devices, hemodynamics and cardiorenal interaction in heart failure, as well as non-invasive diagnosis of rejection in heart transplantation. He has participated in many single and multi-center trials and has published several articles in peer-reviewed journals. He is a Fellow of the American College of Cardiology.

He subsequently went on to train in Internal Medicine and Cardiovascular Disease at SUNY Stony Brook in New York and was elected resident and fellow of the year several times. He then completed his training in Clinical Cardiac Electrophysiology at Massachusetts General Hospital Harvard Medical School and was appointed Clinical Instructor in Cardiology at Harvard Medical School in 2001 as well as SUNY Stony Brook. He has six American Board Certifications. He is currently American Board of Internal Medicine Certified in Cardiac Electrophysiology, Advanced Heart failure and Transplant Cardiology, Cardiovascular Disease , Internal Medicine, and Board Certified in Echocardiography and Nuclear Cardiology. He has always been heavily involved in teaching and been on the teaching faculty at Brown University since 2006. He was promoted to Clinical Assistant Professor of Medicine and Cardiology at Brown University Warren Alpert School of Medicine in 2013 . Dr Sabbour has been active in research and is currently an active PI in two international multi-center trials in the field of pacing and ICDs and has had several publications in the field .He was also appointed the Internal Medicine residency program director at Al Ain Hospital as well as Arab Board Cardiology fellowship site co-director . He has been practicing in Rhode Island for 11 years and moved to the UAE in 2013 to be on staff as Consultant Cardiologist at SEHA (SKMC, Mafraq, Al Ain Hospital and Tawam) Hospitals , he recently moved to CCAD in 2016 . His main clinical interests and expertise are in the management of Arrhythmias & Ablation of Atrial Fibrillation , Advanced CHF and Pulmonary Hypertension and Adult Congenital Heart Disease , as well as familial hyperlipidemia and complex lipid disorders .He is a co PI in the Gulf Familial Hyperlipidemia Registry and co-author on the Middle East Dyslipidemia Guidelines as well as the Gulf ICD Registry and Practical Guidance on Anticoagulation for Atrial Fibrillation in the Middle East .

Richard C. Ferrer, BSN RN CHFN is an Advanced Heart Failure and Transplant Nurse Coordinator of the Advanced Heart Failure and Transplant Program in Cleveland Clinic Abu Dhabi. He graduated in 2011 as Magna Cum Laude at Our Lady of Fatima University in the Philippines and subsequently, worked as a staff nurse in a Medical – Surgical Unit that caters mostly cardiovascular cases at Fatima University Medical Center and has been most interested in the fields of Heart Failure and Electrophysiology.

He was then appointed as Head Nurse of the said unit after 3 years of service. He joined Cleveland Clinic Abu Dhabi as a staff nurse in the Cardiothoracic Unit in 2015 and has been an active member of the Heart Failure Program Committee since then. As Nurse Coordinator of the Advanced Heart Failure and Transplant Program, he is responsible for the coordination of the clinical care for Heart Failure patients and also involved in the evaluation of patients being considered for advanced Heart Failure therapies such as Mechanical Circulatory Support and Heart Transplantation. He has achieved the designation as a Certified Heart Failure Nurse (CHFN) given by the American Association of Heart Failure Nurses Certification Board (AAHFN-CB) in September 2017. He is the first and currently, the only nurse outside the North America to achieve this subspecialty nursing certification.

Tuition Information

The tuition for the HF Academy is AED 8,000.

Full payment must be received before trainees go onsite for experiential training in order to guarantee participation. The tuition fee does not include travel accommodation for experiential training, which is the responsibility of the trainee.

Cleveland Clinic Abu Dhabi is accredited by the Accreditation Council for Pharmacy Education as a provider of continuing pharmacy education and complies with the Accreditation Standards for continuing education activities. The Universal Activity Number for this activity is 0590-0000-19-007-L01-P. This is a certificate program is developed for pharmacists for a total of 30.0 contact hour (3 CEU). The activity will be released October 13, 2019 and expires October 13, 2022. Statements of completion will be issued online as the full program is completed with a post-test score of 80% or higher. Completion of an online evaluation tool is required to receive the Statement of Credit. This information will be automatically reported to CPE Monitor for US pharmacists. Evaluations must be completed at the end of the program. Failure to complete the evaluation by the end of the program will result in a loss of continuing pharmacy education credit.
